Screening questionnaires

PHQ-9, PHQ-2 and the adolescent PHQ-A, GAD-7, SCARED in its child and parent versions, and the Pediatric Symptom Checklist as PSC-35 and PSC-17. Each is reproduced exactly as published, with permission from whoever owns it. The SCARED and the Pediatric Symptom Checklist are English only, because neither permission covers translation.

We are working through permission with the publishers of several more, including the Vanderbilt ADHD scales, the SWYC and the CRAFFT. Ask us where any instrument stands and we will tell you plainly.
